Evaluation Options for 05-06

In order to increase response rates, some faculty members offer incentives to their students, such as small amounts of extra credit.  If you need to receive a list of which students have completed your survey, please complete the form below.

PLEASE NOTE:

1. During the evaluation, students are asked whether or not they would like their name released to the instructor.  Only students who release their name will be included on the list provided to you.  For example, if 25 students complete the survey and only 23 release their name, the report you receive would contain only the names of the 23 students who released their information.  However, the summary of results you will receive the following semester would include the ratings of all 25 students.
    
2. The survey ends on Sunday, December 11, 2005, at 11:59 pm.  Respondent lists will be sent to those faculty who have requested lists no later than Friday, December 16, 2005, at noon.
    
3. Lists will only be sent to PUC email addresses.  Therefore, the email address you provide needs to include @calumet.purdue.edu.
    
4. Requests cannot be processed without a valid CRN.
