Paper Process

For faculty who wish to administer their end-of-course surveys in their courses.  Should follow these steps (click here to print out the instructions):

  1. Complete the form to opt out of the centrally administered process.
     
  2. Print the cover sheet and either the questions as a PDF, if you are only using the core eight questions, or as a Word document, if you would like to add your own questions.
     
  3. Make enough copies of the questions for each of the students in your class. 
     
  4. Obtain enough copies of the standard ScanTron score sheets for each of your students.  These can usually be obtained through your department secretary.
     
  5. Complete the information requested on the cover sheet.
     
  6. Obtain a large campus mail envelope and address it as follows:  CTIS Operations / Course Evaluations / Powers 218
     
  7. Place the completed cover sheet in the campus mail envelope.
     
  8. When you begin to administer your course evaluation, write the CRN for your course on the board for the students to reference or provide it to them in some other manner.
     
  9. Explain the process to your students, highlighting the importance of their feedback and what you plan to do with the information.  Make sure you inform them that you will not review the information until final grades are submitted.
     
  10. Ask a student to distribute the question and score sheets to each of the students in class and drop off the completed evaluation in campus mail.  Make sure you inform the student as to the location of the closest campus mail slot.
     
  11. Students should "bubble" in their answers on the score sheet with pencil (ink and markers cannot be read by the scanner) and return them to the envelope.
     
  12. The designated student should confirm that all score sheets are in the envelope and deposit the envelope in the closest campus mail slot.

CTIS Operations will run the forms as soon as the processing of finals are complete.  You can expect to see the results in about two to three weeks after submission.

You will be sent the results in two forms.  First, you will be provided a histogram of the response for each question (click here to see a sample).  Second, you will receive a statistical analysis for each question (click here to see a sample).
